Mumbai: With the monsoon settling in, the water borne diseases have started hitting the city hard. OPDs in hospitals are packed with patients suffering from diseases like gastroenteritis, cholera, jaundice and hepatitis A.

A spokesman for Regional Public Health said visiting had been restricted to the complex today after reports of vomiting and diarhoea among residents.

Mr Warick Dunn, chief executive officer at Wellington Masonic Villages Trust, confirmed a number of residents were suffering from vomiting and diarhoea at the facility.
"We have followed Department of Health guidelines and shut the facility to any new admissions or visitors … those residents who are unwell have been placed in isolation… and following Department of Health guidelines samples from those unwell have been sent off for analysis to determine what the cause may be.
"Until we get those results we are erring on the side of caution and care for our residents by restricting admissions to the facility," Mr Dunn said.
He declined further comment.
The Dominion Post understands there are at least 12 people affected by the outbreak.

Bakterien und Viren lassen uns dieser Tage keine Ruhe. Dutzende Patienten, Ärzte und Pflegepersonal haben sich am Münchner Klinikum Rechts der Isar mit dem Noro-Virus angesteckt. Das Krankenhaus gab am Mittwoch jedoch bereits Entwarnung. Die Krankheit verlaufe „sehr mild“, sagte eine Sprecherin. Derzeit litten nur noch drei Patienten und drei Mitarbeiter an dem Magen-Darm-Virus.
Am Dienstag waren noch 45 Patienten sowie 20 Beschäftigte betroffen, vergangenen Freitag gab es sogar 80 Erkrankte. Nach Angaben der Sprecherin hat vermutlich eine Krankenschwester den Virus eingeschleppt. Nach Ausbruch der Krankheitswelle seien umgehend die Hygiene-Maßnahmen verschärft worden. (dapd-bay)

How Seattle’s Paid Sick Days Requirement Would Work | The SunBreak
First of all, the Seattle City Council is not yet ready to vote on the paid sick day legislation; the Housing, Human Services, Health, & Culture committee meets today, June 22, at 2 p.m., to discuss the details. Committee chair Nick Licata hopes to have the full Council vote on his proposal by August. If passed, it would go into effect a year later, and then businesses, depending on size, would have 90 or 180 days to begin implementing paid sick days for their employees. Businesses with fewer than 250 employees get a longer runway.
Seattle’s proposed legislation is one of a number of moves in cities and states around the country to require businesses of all sizes to provide paid sick leave. HuffPo: “There are active advocacy campaigns in about 20 cities and states, and bills are progressing in a number of states including California and Massachusetts.” Notably, San Francisco passed a requirement in 2006. Connecticut took the state-wide lead this June.
Seattle’s proposal differs from many others in key respects. The ordinance would apply to businesses of all sizes, but in a tiered manner, so that truly small businesses are not unduly burdened. The Seattle Times breaks out the sick day levels:
- A business with 1 to 49 full-time employees would have to provide up to five days of paid sick leave in a year.
- A business with 50 to 249 full-time employees would have to provide up to seven days in a year.
- A business with 250 or more full-time employees would have to provide up to nine days in a year.
- Businesses with more than 1,000 employees would have to provide one hour of paid leave — which might include vacation time — for every 15 hours worked, with at least half the time available for paid sick time.
Crucially, after feedback from small business owners, the proposal preserves the ability of employees to swap shifts, rather than take sick days preferentially. Businesses that already provide paid time off, in hours greater than or equal to sick leave requirements, could count that time as paid sick leave.
Employees accrue sick leave depending upon hours worked, so part-time and full-time workers are covered. The range is from one hour of leave per 50 hours worked for the smallest businesses to one hour per 15 hours for the largest.
The Seattle Coalition for a Healthy Workforce estimates about 190,000 Seattle workers currently do not get sick days, that would. (The 2002 census counted almost 65,000 businesses in Seattle.)
Despite employers’ fears about abuse of sick day leave, nationally, the average actual sick days taken is 3 days annually, rather than the maximum. In San Francisco, “Average use across all industries was 3.3 days, with an average of 1.9 days in the leisure and hospitality sector – in which restaurants and bars provide the majority of employment.” PubliCola reports that in San Francisco, three years after implementation, “68 percent of all employers, and 66 percent of hotel and food service employers, support the ordinance.”
A Connecticut study on the costs of extending sick leave (to firms with 50 employees or more) pegged the expense as .05 percent of total sales–without taking into consideration the costs associated with workers showing up sick, and spreading the flu, for instance, around the workplace.
Dr. Jeff Duchin (Communicable Disease & Immunizations, Public Health, Seattle & King County) notes that: “In King County, from 2006 to 2010 approximately 30 percent of recent food borne illness outbreaks (almost all due to norovirus) were linked to food handlers who worked while ill.”

Holidaymakers, who were struck down by a highly contagious stomach bug at a holiday camp in Bognor Regis, have launched a claim for holiday compensation.
An estimated two hundred campers at the Butlin’s Holiday Camp in Bognor Regis, West Sussex were affected by the norovirus outbreak and many went on to develop a nasty bout of gastroenteritis, symptoms of which include nausea and diarrhoea.
Forty three year old Kathleen Gilmovitch, of Princes Road, in Ellesmere Port, Cheshire, became a victim of the virus after spending just three days at the camp. She told the Ellesmere Port Pioneer that she had to fall out of bed and literally crawl to the bathroom, because her stomach cramps were so bad.
Butlins have since offered a full refund to guests affected by the illness and have also offered alternative accommodation to future guests at Skegness and Minehead if requested.
A spokesperson for Butlins stated that measures had been put in place to stop the virus spreading, adding that cleaning and sanitising procedures had been stepped up in an attempt to interrupt the natural spread of the virus.
Arun District Council’s Environmental Health team said that the norovirus bug is easily spread especially when there are lots of people in close proximity; they went on to say that a full investigation had been launched in partnership with the Health Protection Agency.

"The National Water Services Commission (SPAN) has urged all users of individual septic tanks to be more concerned with pollution from sewerage which could cause the spread of water borne diseases such as hepatitis A, cholera, typhoid fever can threaten the health of the people," she said.
Therefore, she urged SPAN and Indah Water Consortium to continue organizing awareness campaign to tell the people the importance of desludging individual septic tanks at least once in three years, since it would cost only RM6 per month for three years.
She was speaking to reporters after officiating a national level Campaign to Desludge Individual Septic Tanks at Kampung Kenangan Tun Dr Ismail 2 here today.

Norovirus Teaches Hard Lessons to a Retirement Community : Family Practice News
Retirement community staff should prepare themselves for norovirus outbreaks, which can shut down dining halls, damage public relations, and strain residents’ physical and mental health, according to staff members of a Portland, Ore., facility who learned the hard way last year.
"Knowing what we know now, there are a lot of things we would have done differently," said Mjere Simantel, director of social services at Willamette View, a continuing care residential community in Portland, at the annual conference of the American Society on Aging.
Norovirus, which causes diarrhea, vomiting, and fever, is seldom deadly and most patients recover in 48 hours, but it can spread quickly in the tight quarters of a retirement community. And victims can continue shedding the virus for weeks after symptoms fade.
The virus had visited Willamette View before, but without doing much harm. "Previous outbursts were taken care of very quickly," said Rikki Schoenthal, community counselor for the 500-bed facility.
She first became aware of last year’s outbreak in March of 2010 when some residents complained of food poisoning. That was on a Friday, and the local health department was closed because of furloughs. Over the weekend, the number of residents with symptoms began mushrooming. The health department ordered the community to close the dining room and stop cross-traffic between the community’s buildings.
"Our dining service had to figure out how to feed our residents," recalled Ms. Schoenthal. "How were we going to deal with the hair salon, the computer lab, the laundry room, the health center, the pool, the bank, the cleaning room, the library?" Eventually, almost all social activities were canceled.
And the staff found themselves on a sharp learning curve. One hard lesson was that the virus can live on ordinary surfaces indefinitely. "We actually had staff wiping down books," said Ms. Schoenthal.
Another lesson: Hand sanitizers don’t kill the virus. It has to be physically removed from skin with soap, water, and scrubbing. Likewise, vacuuming carpets can send the virus airborne. The community invested in a large stock of chlorine wipes, only to find them ineffective. Some chemicals that were used to sterilize surfaces proved so potent they took the paint off walls, corroded doorknobs, and damaged elevator buttons.
With such confusion, communication proved very important. The management slipped newsletters under each resident’s door every day. Still, residents felt isolated; the only human contact for some was the staff who delivered their meals.
"Some of the residents felt like it was room service," said Ms. Schoenthal. "Others reacted with dismay. They felt like they were being ‘quarantined.’ That word started to be used and we did not want it to be used."
Staff was strained to the breaking point. Those whose work was involved with social activities had to be reassigned. Dining hall staff had to work extra hours. Even the CEO began delivering meals.
The meal deliveries became the community’s main means of checking on the daily health of each resident. The management created a database to track who had fallen sick and who had recovered. Eventually, 96 residents got sick and 2 were hospitalized with dehydration.
Some 50 staff members fell ill as well. "Some of the older adults had accidents, and the environmental staff had to go in there and clean up, and most of them got sick," said Ms. Schoenthal.
She saluted their sacrifice. "The way the staff protected the dignity of the residents in an undignified situation was just stellar," she said. Although the management recognized this effort with a $50 bill for each staff member, many staff members suffered financially because they were forced to take time off after falling ill with the virus. Those who didn’t have enough paid days in their account had to take the time off unpaid. "That was something we still want to explore," said Ms. Schoenthal.
As the outbreak stretched on, the strains increased. The health department insisted that the dining hall be kept closed for 4 weeks, and no visitors or social activities were allowed.
"We had residents who didn’t leave their rooms for 4 weeks," said Ms. Simantel. "Our residents were used to being extremely social. They went from all to nothing. It felt to them like a lockdown, and we still have residents report that they have not recovered."
The staff piped brain teasers into the residents’ rooms through closed-circuit television and began making phone calls to them, but many residents suffered from the isolation. One man had to be referred to hospice care when the restrictions were ended.
The Willamette presenters recommended the following issues for similar communities to consider before they’re hit by norovirus:
• When to activate an emergency plan.
• How to collect data.
• With whom to share the data.
• What role state agencies will play.
• Whether staff has to use paid time off if they get sick from their work.
• What your business insurance will cover.
• How to clean infected rooms.
• How to communicate with state agencies and the news media.
• Whether to use the "Q word" (quarantine).
The presenters also listed the following measures that they would do differently:
• Broadcast a closed-circuit television program of physical exercises.
• Ask health department experts to visit and make recommendations tailored to the community’s circumstances.
• Allow visitors to go to residents’ rooms while wearing gowns, masks, and gloves, which they would discard after each visit.
And audience members who have experienced norovirus outbreaks offered the following additional suggestions:
• Keep a box of supplies on hand for such an emergency.
• Put magnets on the doors of sick residents so that staff can tell which is which.
• Create a system of phone friends to help break the isolation.
Whatever else they do, staff members should be prepared for a longer haul than they expect, said Ms. Schoenthal. "We just kept thinking it would end tomorrow."

Hepatitis E claims 10 lives in Sambalpur - southindia - Orissa - ibnlive
Hepatitis E virus has claimed 10 lives in the last one week triggering panic in Sambalpur town.
The deceased include two pregnant women. Medicine Specialist Kabir Das Purohit said it develops into a severe liver disease in some cases.
He said while 90 per cent of the patients recover, it may lead to complications in cases of pregnant women, elderly patients and those addicted to alcohol, tobacco and drugs.
There is 20 per cent mortality rate among pregnant women, especially those in the third trimester. The disease mainly spreads through contamination of water supplies or food with faeces, Purohit said.
Chief District Medical Officer B P Guru said that unconfirmed reports of eight to 10 deaths have been received by the medical administration.
He said the ADMO (public health) has been directed to take measures to contain the disease. “Source of infection will be determined and provisions for supply of safe drinking water and proper sanitation will be made”, he said.

Une plate-forme de génotypage bactérien pour Staphylococcus aureus
Le département R&D de CEERAM a développé une méthode de génotypage bactérien pour S. aureus par MLVA. A l’instar des techniques utilisées par la médecine légale pour l’élaboration d’empreintes génétiques humaines, cette méthode d'analyse s’appuie sur l’étude concomitante du polymorphisme de plusieurs VNTRs, séquences répétées en tandem caractérisée par une variabilité de répétition intra-espèce.

Epidémies d’E.coli : tirons les leçons des crises [ 20.06.2011 ]
Les consommateurs sont à juste titres inquiets suite à l’hospitalisation de sept jeunes enfants victimes d’une intoxication alimentaire grave probablement due à la consommation de steak hachés porteurs de la bactérie Escherichia Coli (E. Coli).
Cette inquiétude est légitime car ces 6 derniers mois, les problèmes sanitaires liés à l’alimentation se sont multipliés. Depuis janvier, les consommateurs ont ainsi été informés de la présence de dioxine dans la viande allemande, de manquements graves à l’hygiène dans des fast-food qui pourraient avoir entraîné deux décès, d’une contamination de tomates séchées par le virus de l’hépatite A et plus récemment de deux épidémie liées à E. Coli, l’une dans des graines germées allemandes, la seconde dans des steaks hachés.
Bien sûr, l’actualité immédiate ne doit pas occulter la tendance de fond à savoir que les intoxications alimentaires ont fortement diminué au cours des dernières décennies. Cependant, la sécurité alimentaire est un défi permanent qui appelle une rigueur et une vigilance continue de tous les acteurs : professionnels, pouvoirs publics et bien sûr consommateurs.
Aujourd’hui, nous manquons d’information sur les moyens mis en œuvre par les autorités sanitaires en matière de surveillance de la chaîne alimentaire. Quels sont les fréquences et les résultats des contrôles dans les différents secteurs de l’industrie agroalimentaire ? Quels sont les moyens humains et matériels dans ce domaine ? Quelles mesures sont adoptées pour améliorer la situation ?
Faute d’indicateurs et de chiffres, ces questions essentielles restent en suspens et il est impossible de suivre objectivement l’évolution de la pression de contrôle sur les professionnels.
Pourtant, les contrôles officiels sont un déterminant majeur de la sécurité alimentaire, complémentaire des procédures de gestion des risques mises en œuvre par les industriels eux-mêmes. En matière sanitaire, la responsabilisation des professionnels est évidemment bienvenue mais elle ne saurait suffire à garantir la fiabilité des denrées aux yeux des consommateurs. Afin de préserver la confiance de ces derniers, nous demandons la publication annuelle d’un tableau de bord des contrôles réalisés par les administrations et des résultats obtenus. La CLCV invite les parlementaires à inscrire une disposition en ce sens dans le projet de loi en faveur des consommateurs actuellement examiné par l’assemblée nationale.
Food, water contamination blamed for gastro cases in Bangalore - Health - DNA
Gastro cases have doubled up in the city over the past one month, said Dr Tejasvi Nataraj, consultant, internal medicines, BGS Global Hospital. The doctor is seeing around four to five cases every day.
“Mostly, this is due to food contamination and sometimes water contamination. Usually, it is taking two to three days for the patient to recover,” said Dr Tejasvi. It is better if people avoid eating out this season to prevent gastroenteritis (GE), he added. Not just GE, contaminated water can transmit other diseases like viral, hepatitis, gastroenteritis, cholera and typhoid. “Illness from acute gastroentritis can range from mild annoyances to devastating dehydrating illnesses that can kill a victim in hours. Acute gastrointestinal illnesses rank second only to upper respiratory infections as most common illnesses worldwide,” said Dr Srinivasa Devashetty, consultant, Fortis Hospital.
“In recent years, there has been a 10-fold increase in Viral Hepatitis A and E. These are transmitted by contaminated water, inadequately boiled milk, salads, shellfish and frozen fruits,” he added...

Hepatitis E sounds alarm
Waterborne liver diseases have appeared as a major public health concern in the country, where safe drinking water remains a pipe-dream for many, experts warn.
The warning followed the lab confirmation of hepatitis E outbreak in the Kumudini Welfare Trust's Mirzapur complex in Tangail, a neighbourhood of the capital Dhaka.
A number of students residing in dormitories of the Kumudini Medical College and Hospital, Nursing School and Bharateswari Homes were taken ill early June with yellow coloration of eyes and skin.
The Institute of Epidemiology Disease Control and Research (IEDCR) swung into action following the illness and their repeated tests confirmed the virus to be hepatitis E on Sunday.
"We also identified water as the source of infection," IEDCR director Prof Mahmudur Rahman told bdnews24.com on Monday.
Analysis of acute to chronic hepatitis E
Hepatitis E as a self-limiting disease and is generally not chronic, although in a small number of cases it appears to be so. Complete data were collected from 1104 cases of hepatitis E out of which 28 did not heal after liver treatment. These 28 cases were followed-up for 6-10 years. It was found that hepatitis can be chronic. Chronic hepatitis cases are more common in males, but a high degree of inflammatory activity and fibrosis were not evident. After 6-10 years of follow-up observation, cirrhosis or liver cancer did not appear. This is significantly different from the hepatitis B and hepatitis C viruses. Further, most cases of chronic hepatitis E occurred in older patients, in which the ability to remove the virus may decline, contributing to the more than six months duration, along with other important reasons. In addition, we also observed that several cases were unhealed or chronic because of a combination with other viruses. Chronic disease or the long-term use of immune agents may be one of the causes for chronic hepatitis E.

Cette législation détermine des paramètres applicables aux eaux conchylicoles désignées, ainsi que des valeurs guides, des valeurs impératives, des méthodes d'analyse de référence et la fréquence minimale d'échantillonnage et de mesure.

Ceeram -Microbial Source Tracking & Directive 2006/7/CE
La nouvelle directive européenne (2006/7/CE), adoptée le 15 février 2006 par le Conseil de l'Union européenne et par le Parlement européen, concerne la qualité des eaux de baignade dans les États membres de l’Union Européenne (UE) (sauf eaux de piscine et eaux destinées aux usages thérapeutiques).

Ce texte prévoit la manière dont les Etats membres vont :
• surveiller et classer la qualité des eaux de baignades,
• gérer la qualité des eaux de baignades,
• fournir les informations au public.
Profil des eaux de baignade
La gestion des eaux de baignade sera au préalable fixée par la réalisation d'un « profil » des eaux de baignade par les communes et les gestionnaires privés d’eaux de baignade. Ce profil correspond à une identification et à une étude des sources de pollutions pouvant affecter la qualité de l'eau de baignade et présenter un risque pour la santé des baigneurs. Il permettra de mieux gérer, de manière préventive, les contaminations éventuelles du site de baignade.

La directive prévoit que seuls 2 paramètres microbiologiques seront à contrôler (Escherichia coli et entérocoques) à raison de 4 prélèvements au minimum par saison balnéaire. En fonction des résultats des analyses effectuées sur une période de 4 ans et selon une méthode de calcul statistique, les eaux de baignade seront classées selon leur qualité : insuffisante, suffisante, bonne ou excellente. Les normes de qualité, différentes pour les eaux de mer et les eaux douces, exigent une qualité d’eau au moins suffisante pour l’ensemble des eaux de baignade.

L'information et la participation du public est primordiale. Le public informé devient alors acteur dans la gestion de la qualité des eaux de baignades.

Quelques dates à retenir :
• Les profils des eaux de baignade sont à établir au plus tard en 2011.
• Le premier classement basé sur 4 années de contrôle doit être établi au plus tard à la fin de la saison 2015.
• Toutes les eaux doivent être au moins de qualité suffisante à la fin de la saison 2015.
• Révision de la directive au plus tard en 2020, sur la base des résultats d'études épidémiologiques, de recommandations de l'OMS, des progrès scientifiques et des observations des Etats membres de l'Union européenne.

Cruise delayed after norovirus sickens 142
Princess Cruises says a total of 142 people became ill during the ship’s May 30th-to-June 9th cruise, which visited Juneau, Skagway, Glacier Bay and Ketchikan.
The San Francisco-based ship’s next departure was pushed back while crews disinfected the Sea Princess. Spokeswoman Karen Candy says the delay was just four hours. But it was enough to force route changes.
Sitka gained an unscheduled stop, Juneau saw the ship later than expected and Haines lost a docking.
The Sea Princess carries about 2,000 passengers and 900 crew members during 10-day Inside Passage cruises. Other stops along the route can include Skagway, Tracy Arm and Victoria, British Columbia.
Norovirus, also called Norwalk-like virus, is a common gastrointestinal illness that causes diarrhea and vomiting. Princess Cruises says it followed U.S. Centers for Disease Control protocols in reporting the illness and cleaning the vessel before its latest sailing.
The cruise line says norovirus numbers were also higher than usual during the ship’s two previous trips.
CDC inspected the Sea Princess on May 24th and identified 13 problems, including a leaking pipe at a soup station. However, all the issues were considered minor and the overall health rating was high.

Bactérie mortelle E.coli : trois nouveaux décès portent le bilan à 33 morts

BERLIN — La bactérie E.coli (Eceh) qui peut provoquer des diarrhées mortelles a fait trois nouvelles victimes en Allemagne, ont indiqué vendredi des autorités sanitaires régionales allemandes, ce qui porte le bilan de l'épidémie à 33 morts, dont 32 en Allemagne.
Dans la matinée, le ministère de la Santé de Basse-Saxe (nord) avait annoncé le décès d'une femme de 75 ans.
Et au cours de l'après-midi, les morts d'une septuagénaire de Schleswig-Holstein (nord) et d'un homme de 81 ans, hospitalisé à la clinique universitaire d'Hambourg-Eppendorf (nord), ont été rendues publiques.
Les autorités sanitaires fédérales ont annoncé vendredi que la source de la contamination par une souche rare et très virulente de la bactérie E.coli entérohémorragique (Eceh) était des graines germées.
La souche virulente et résistante à l'oeuvre en Allemagne a même été identifiée, pour la première fois, sur des graines d'un sachet qui provenait de la ferme biologique Gärtnerhof à Bienenbüttel (nord), soupçonnée depuis plusieurs jours d'être à l'origine de la contamination.
La bactérie mortelle a été identifiée par des scientifiques de Rhénanie du nord-Westphalie (ouest) dans un sachet retrouvé dans la poubelle d'une famille dont deux membres avaient été malades.
Les autorités sanitaires allemandes ont du même coup levé l'alerte à la consommation de concombres, de tomates crues et de salades qui était en vigueur depuis le 25 mai.
Les graines germées de lentilles, luzerne, soja, trèfle rouge, moutarde, blé, radis, fenouil, etc., riches en vitamines et minéraux, sont réputées pour leurs qualités nutritionnelles. Devenues populaires ces dernières années en restauration, elles sont servies notamment dans les salades et les sandwichs.
Cultivées dans l'humidité et à des températures de plus de 20°C, elles ont également été identifiées par des chercheurs comme des vecteurs de bactéries pathogènes, comme l'Eceh, la salmonelle ou la listéria.

norovirusuitbraak - Meer aandacht voor buikgriepvirus op cruiseschepen [gezondheid.be]
Het Nederlandse RIVM pleit voor meer controle op cruiseschepen op voedsel dat besmet is met het norovirus dat buikgriep veroozaakt. Op cruiseschepen komen zeer geregeld uitbraken van norovirus voor. Het norovirus veroorzaakt maag- en darmklachten en komt veel voor. Gezonde mensen zijn binnen drie tot vier dagen weer beter. Bij ouderen of mensen met een zwakke gezondheid kan het virus een ernstiger verloop hebben.
Een op de vijf uitbraken ontstaat door besmet voedsel. Het virus verspreidt zich via voedsel dat niet wordt verhit, zoals rauwe oesters of frambozen. Het voedsel is dan in contact gekomen met water dat is verontreinigd met ontlasting.
Volgens een onderzoek van de Amerikaanse Centers for Disease Control and Prevention is de grootste risicofactor om tijdens een norovirusuitbraak op een cruiseschip zelf ook ziek te worden, het hebben van een zieke hutgenoot.
Volgens de onderzoekers moeten zieke cruisepassagiers zich zo snel mogelijk melden bij de ziekenboeg, zodat het personeel in actie kan komen (isolatie, rigoureus schoonmaken, betere handhygiëne) om een uitbraak te voorkomen.
